NZQA approves entitlement to a derived grade to students sitting in external exam who meet the eligibility criteria. Applications are made to NZQA by the school on behalf of the student.

Forms must be completed in full and will require a medical practitioner to complete section C. The Derived Grade Form is attached to this article.

Applications must be submitted to the School before the 5th December to meet the closing time of NZQA. Scanned copies can be sent to assessment@burnside.school.nz or paper copies can be left at the Student Office
