Caregiver Consent Agreement For School Trips

Now that we are in COVID Alert Level 1, school trips may start to occur again. In order for students to participate in these, we need every parent/caregiver to complete this consent form. Many did this before lockdown, but those who did not will need to complete this form by Friday 19th June. If you have already completed this form, you can disregard this email.

Could you please read this Agreement and complete all sections of this form by Friday 19th June 2020. 

As a school, we are streamlining our consent and notification process for student trips. It is important that there is clear communication between the school and home and that you are aware of when your daughter is leaving the school grounds. In all instances where your daughter is leaving the school on a trip, you will be notified well in advance, however we are looking to reduce the number of times you need to give permission for these trips. This consent is to cover events which occur during the course of a school day and conclude prior to approximately 6pm. Examples of these may include class trips to town which take place during one period, sports events to Pegasus that take half a day, or assessment related trips away to university/beach/museum for the whole school day.

Where an event involves risk exposure greater than what would typically be the case at school, such as adventurous activities or hazardous environments, or the event continues overnight, specific consent will be required. At the time of our seeking any further consents you will also be asked to update the health and contact information held by school.

It is important that consent is given at the start of their time at Christchurch Girls’ High School for all students who will be participating in education outside the classroom (EOTC) events as described above. Details given will remain confidential to those supervising activities on EOTC events. It is crucial that you provide us with up to date information, that is accurate and complete, to allow us to plan appropriately for EOTC events.

Please note that is very important that student details such as health information and emergency contacts are kept up to date with the school office during the year.
