Dear Parents / Caregivers,Public health nurses will be delivering two school-based immunisation programmes in yourchild’s school during 2023.

Year Seven StudentsThese students will be offered the Boostrix vaccine which is a booster for their earlychildhood immunisations of tetanus, diphtheria and pertussis (whooping cough). This is thesame vaccination that would have been given to your child at 4 years age if you consented.Information about the year seven programme will be given to students at school and thissession may include watching the Ministry of Health Presentation on Year SevenImmunisations and is designed specifically for this age group.

At this information session, your child will be given a consent form to bring home for you toread and choose whether or not to have your child immunised by the Public Health Nursesat school. Whether you choose to immunise or not, the form needs to be returned to school.

YEAR 7 IMMUNISATIONThe Yr 7 immunisation for Diphtheria, Tetanus and Whooping Coughwill be undertaken during 2023.

Consent forms will be sent home with EVERY Yr 7 student.

Please complete and return to the School Office.

There is provision within the form for your non-consent, should you wish for that.

PLEASE REMEMBER- WE WOULD LIKE ALL FORMS RETURNED REGARDLESSOF IF THEY ARE FOR CONSENT OR NON-CONSENT.

As part of the programme information will be given to each Yr 7 student.If you do not want your child to receive this education and/or to view the presentation the Public Health Nurse uses, then send a letter to the Office so that your child can be removed from that class presentation on the day of the education session.

Year Eight StudentsFrom 1 January 2017, Males and Females aged 9 to 26 inclusive (ie under 27 years) havebeen eligible for HPV (Human Papilloma virus) immunisation. The focus has historicallybeen on cervical cancer prevention for women but increasing evidence has emerged of theimpact of other HPV related cancers on men so both groups are now offered this vaccinefree of charge. This vaccine will be given in two doses 6 months apart (March/April andOctober/November).

Information will be given to the students about the HPV vaccination programme at school.This may include viewing the Ministry of Health presentation about the HPV VaccineInformation and is for the year 8 age group.

Your child will be given a consent form to bring home for you to choose whether ornot to have your child immunised by the Public Health Nurses at school.

 Information on the immunisations is included on the consent form which will be givento your son / daughter at the information session.

 Please complete the consent form and answer all the questions and return the formto school.

 If your child is not being vaccinated at school you still need to fill in the section withyour child’s name and details on it.

 Please check with your doctor or practice nurse if you are unsure if your child hashad a tetanus injection in the last two years.

YEAR 8 IMMUNISATIONThe Yr 8 Immunisation for HPV (Human Papilliomavirus) which can develop intoCervical cancer and other HPV related cancers, will be undertaken in two doses during 2023.Consent forms will be sent home with EVERY Yr 8 student.
