The Southern District Health Board has been pulling together information on community resources.

This is part of a Home as my First Choice campaign, supporting people to remain in their own homes and communities for as long as possible. There is now really useful information for all of us who at are contact with people in the community who may be struggling to remain in their own homes. There is a printable version of these resources attached below.
