We can now accept new forms created by the Ministry of Business, Innovation and Employment (MBIE) for use when inspection, maintenance and reporting procedures are missed.

The forms are the BWoF Report and Declaration (B-RaD) and Specified System Report and Declaration (S-RaD) forms.
