As part of the Greymouth High School bi-annual community survey on Health Education, we’re inviting parents and community members to give feedback on:

1. Our draft health education delivery statement, and
2. The overview of the health education programme.

This feedback is extremely valuable towards our Health programme, and also allows us to:

• refine and revise Health Education programmes based on community needs
• assist in the development of well-being related policies
• provide opportunities for parents, students and whānau to contribute to and enhance learning in Health Education
• raise the profile of Health Education within the school and community

Your feedback is anonymous, and important to us. The information you need to answer the survey is provided. The survey asks you to rate your level of agreement with the documents and/or provide comment, plus some general comments on the programme itself.

The survey should take no more than 5 minutes.
