One of the things we require our parents/caregivers to do is to complete a Blanket Consent Form. Blanket Consent is when you give consent, by signing the form below, for your child/children to participate in low-risk activities. Please note, you will always be informed of any event that is planned and when that event will occur. Usually this is completed on enrolment but as this is a completely new form we are required to use, we are asking all parents/caregivers to do this by the end of the term. This is a one off. You won't be asked to do this again.
