IMPORTANT REQUEST FOR ALL FAMILIES

Please ensure that we have up to date evidence / proof / documentation to show that your child is fully immunised, or a letter saying you have chosen NOT to fully immunise your child.

If we do not have up to date evidence or a letter stating you have chosen not to fully immunise your child, you then accept that your child will have to stay at home in the event of an outbreak ( e.g. measles which is a current problem).

INTERNATIONAL FAMILIES:

It can be difficult interpreting immunisation documents that have been completed overseas, and we are not certain if your child is fully immunised according to NZ standards. We ask that you please visit your local GP/Practice to ask for this information.

We have provided two photos to show what information is required:

1. From eTap, which is where we hold all student details. 

2. The NZ Immunisation records (found in Plunket book for up to 5 years of age).
