Youth Health Assessment

Adolescence is a time of growth and development and can be challenging for some young people. Early identification of concerns ensures support and interventions can be put in place to assist students in their learning and health journey.  

As part of a comprehensive health check offered to students, the School-Based Health Service Nurse may be using the HEEADSSS Assessment, a validated comprehensive psychosocial interview tool used to evaluate the health and well-being of adolescents, addressing areas such as:

  •  Home
  •  Education/Employment
  •  Exercise/Eating
  •  Activities
  •  Drugs/Alcohol
  •  Sexuality
  •  Suicide/Depression
  •  Safety


The HEEADSSS Assessment provides a structure for:

  • Developing rapport with a young person, while gathering information about their world, including family, peers, and school.  Developing a picture of the young person’s strengths and protective factors.  
  • Conducting a risk assessment and screening for specific risk factors – including drug and alcohol use, mental health risks.  Identifying areas for intervention and prevention.
