Year 8 Parent/ Guardians

Information about Free Adolescent Dental Care for your child was recently sent to you in the post.

Included was a list of dentists and a Year 8 transfer form.

This form needs to be filled in with the name of your child’s chosen dentist and returned in the postage-paid envelope to the School of Dentistry.
