A reminder to parents and guardians to complete and return the MMR Consent form to the school as soon as possible even if you do not wish your child to receive the vaccine, the forms are required to be completed - using the decline section -  and return it to school as soon as possible.

We have recently been into Massey High School and completed the MMR Immunisation talks for Years 9, 12 & 13. A consent form was sent home with your child on the day. If you have not yet received a consent form, spare forms can be collected from the school office, printed from the attachment on this newsletter or you can contact our team on Ph 09 838 1807 and we will be happy to send one out.
