With strict Ministry of Transport and Ministry of Health legislation around contact tracing we require all students who intend to use either the 714 Papamoa East school bus or the Welcome Bay MOE school bus, during Level 2, to fill out this form. 

Please fill the form out for each child in your family intending to use the bus during Level 2.

Please be aware there are very limited spaces on the 714 bus service during Level 2 so if it is at all possible for students to get to school using another form of transport it is strongly encouraged.
