Regional Public Health Immunisation Programme

The Rāroan —

Year 7 and 8 students are eligible for free immunisations. This Monday, our tamariki will be given a consent form to take home for you to read, sign and return to school before they are immunised (blue Boostrix form for year 7s and orange HPV form for year 8s).

Regional public health nurses administer the immunisations and, as a kura, we support the Ministry of Health by providing the time and space for students to get immunised with either their Boostrix or HPV vaccine.

The form gives you the option of stating that you do not give consent for your child to be immunised at school. If this is your preference, please complete Section B and return to school. All forms taken home must be returned to school. The immunisation programme runs to a tight schedule, in order to cover all schools in our region. Please ensure forms are sent back as soon as possible.

If you do not want your child to bring home an Immunisation Form, please complete the following google form prior to Monday lunchtime. If we do not hear from you, your child will be given a form by their classroom teacher on Monday 14 February.
