Immunisations at School

All students should have brought home an immunisation form for completion by their parents & caregivers (blue Boostrix form for year 7s and orange HPV form for year 8s). 

Please arrange for the forms to be completed and returned to the school office as promptly as possible.  The immunisation programme runs to a tight schedule, in order to cover all schools in our region.

If you do not wish your child to be immunised, please complete Section B.

All forms must be returned.
