In response to the Ministry of Health’s mandate we have updated our EOTC Generic consent form.

Whānau completed this EOTC form as part of the 2021 & 2022 enrollment process. If your young person enrolled at Tauranga Girls’ College before 2021 the EOTC generic consent form was brought home to be signed at the beginning of 2021.

We have added the following statement to this consent form.

Covid Protection Framework: Many of our trips will require students to be fully vaccinated and to have a Ministry of Health issued ‘My Vaccine Pass’. The trip organiser will inform students & whānau if a My Vaccine Pass is required.

Every EOTC Trip has an information letter which is sent home via the student who is attending the trip. In this letter there will be information IF a My Vaccine Pass is required. The trip organiser may require proof their My Vaccine Pass before being able to attend the trip.
