Trip permission forms

Upper Harbour Primary School —

We're sharing some of the changes we've made to strengthen our EOTC programmes

At Upper Harbour Primary School, our education outside the classroom (EOTC) programme includes all events that occur outside the classroom, both on- and off-site, including all curriculum-based activities (e.g. trips and camps) and extra-curricular activities (e.g. sports days or cultural events).

When your child participates in an EOTC event you are required to complete a trip consent form for your child to participate. This inform has been updated to reflect information we are required to collect for each trip, including consent, medical information and acknowledgement of risk.

While these forms may initially appear extensive we believe the information they provide strengthens the overall health and safety of EOTC events by helping to identify any reasonable foreseeable risks and hazards, and implementing effective management procedures to eliminate or minimise these.

We plan many exciting trips and events throughout the year and know you will quickly become familiar with these forms.

Should you have any questions about a trip, or feel you require more information, you are encouraged to contact the teacher in charge - listed at the bottom of the form, or make contact with a member of the Senior Leadership Team.

All trip permission forms are shared with parents and whānau via Hero.
