The annual Health and Safety Audit (Loss Prevention Checklist) survey is now open until the end of June

What you need to do...

The Diocesan Council asked that all faith communities return a completed copy of a loss prevention checklist (attached below) by 30 June each year. We are asking Wardens to be the main people to complete this and asking Archdeacons to follow this up at their meetings.

  • To complete this audit electronically, click here (please complete the audit this way if you can). 

  • To complete the audit via paper copy, download and print the form attached below - please contact us if you would like a copy posted to you.

You may like to use a paper copy (below) as a way of gathering information from several people and then responding to the survey on-line. 

If your faith community has more than one location, please complete one of these for each one.
