Allergies, Asthma Plans - Medications, Severe Medical Conditions and Accidents

Please let the office know if your contact details or address have changed. Working together we grow healthy students.

If your phone, email or address changes please inform the office.

  • Accidents - In the event of an accident your child will be administered basic first aid. In the case of serious accidents parents/caregivers will be contacted. In the event a caregiver cannot be contacted the student will be taken to a an After Hours Clinic.

  • Allergies and Severe Medical Conditions - Medications for specific allergies/illnesses will be kept in our Sickbay.  The school staff will only administer prescription medication to students whose legal guardian has provided the school with a Medicine Authority Form (attached)

  • YSTT has an Allergy and Asthma Policy and one of the expectations in this policy is that your doctor will complete and sign an Action Plan (attached) for your child.

  • Medication - An adult must hand all named prescribed medications to the office and a Medicine Authority Form (attached) must be completed and signed by a primary caregiver for the school office. The medication(s) will be stored in the Sickbay and administered by office staff.

  • Medication held in the sickbay must be collected at the end of each year.  At the beginning of each year a new Medicine Authority form is to completed for your child/ren and given to the office with the GP/Pediatrician prescribed medication.

  • It is the primary caregivers responsibility to monitor and replace expired medication.
