Medical Details Update
As part of our care plan for each student, we would like to ensure everyone's medical details are up to date. Please look at the below linked forms and complete anything relevant to your child.
Medical Details Form - for use if your child has a medical condition the school need to be aware of when they are here or away on a school trip.
Medicine Authority Form - for use if your child is to be administered any medication during the school day. This includes any medicine the school holds in case of an emergency for your child.
