Student Records Update Form

It is really important that your child's contact details and medical information are kept up to date.

With upcoming EOTC activities including the Antarctic Centre it is very important that you keep up our staff informed of any changes of medical information or contact details.

We need to be able to contact you during school hours should you child become unwell or is injured whilst at school or on a school trip or activity. 

This year rather than sending home paper forms we have created an new online Update Form for parents to update the details we hold for your child/children in our Student Management System Hero. 

It is vital that we have up to date contact information for Parents/Caregivers and also several Emergency Contacts.

We also need to know any medical or allergy information for your child/ren.   

Please only complete this form if you wish to make changes to the current information held

If you are unsure of the information we hold in Hero please get in touch with our office team who can provide you with a copy of the details in Hero so you can check if they need to be updated.

Medical Room Process for Contacting Parent/Caregivers

If your child is injured or becomes unwell at school our staff will initially contact the Primary Contacts listed by mobile / home or work phone.  It is therefore very important you inform the School Office should your home, work or mobile phone numbers have changed.

Please ensure that the emergency contacts that you have listed for your child/ren are living locally, able to be contacted during school hours, and are aware that they may be contacted to collect your child, in the event that we have been unable to contact you.

We request that you provide us with at least two emergency contacts for your child.

Please ensure that the school is aware of any updated medical and/or allergy information so our staff can be fully aware of your child’s current medical information.   

If your child requires medication or an inhaler during school hours you will need to provide the School Office with the medication and parents/caregivers must complete a separate authority form from the School Office. Medication (incl. inhalers) must not be kept in bags/desks.
